Have you in the past been treated by a doctor or other health provider for a physical condition that may affect or be affected by your full participation at this meditation retreat? If you answered yes to either of these questions, before you return this application, we recommend that you discuss with your doctor or therapist your intention to participate in this retreat.
Do you have any medical, religious, or other vital dietary requirements? If so, please describe in detail. (Please note, we cannot provide food according to each person’s preferences. This question relates only to requirements that impact your health, moral, or religious strictures.)
Other than granting permission to stay on Dharma Fellowship property, the Dharma Fellowship can not be held responsible in any way for the maintenance, support, care or contentment of anyone who is staying on the property. Any person given permission to reside on Dharma Fellowship property, by their acceptance of that permission, forfeits all rights to claim for any injury or personal harm, physical or mental, suffered or incurred while on the property. Everyone asking to stay on the property must sign to this effect. Please Note: The Eco-Kutis are off the grid, meaning that they do not have electricity. A propane heater is available for heating in colder weather. Don't forget a flashlight! Breakfast consists of oatmeal and fruit salad, with toast and yogurt. Lunches are the main meal of the day and is vegetarian/vegan. In the Buddhist monastic tradition, dinner consists of soup. This menu is designed to support your meditative practices and general well-being during a meditation retreat. Tea is available all day.
